Introduction: Digital health technologies and artificial intelligence (AI) are increasingly recognised as important enablers of Universal Health Coverage (UHC), particularly in low- and middle-income countries. However, evidence on how these technologies contribute to improving healthcare accessibility, quality, and efficiency across health systems remains fragmented. Objectives: This systematic review aimed to analyse the role of digital health initiatives, including AI-driven interventions, in improving healthcare accessibility, quality, efficiency, and equity, with particular attention to evidence from India and other developing countries. Methods: A systematic literature review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Studies published between 2015 and 2025 were identified through searches of PubMed, Scopus, and Web of Science. After screening and applying inclusion criteria, 52 studies were included for thematic synthesis. Results: The findings indicate that AI-enabled digital health tools support improvements in disease diagnosis, patient engagement, and healthcare system efficiency. Telemedicine platforms, AI-assisted diagnostic systems, and mobile health applications have expanded healthcare accessibility. National initiatives such as the Ayushman Bharat Digital Mission illustrate how digital health infrastructure can support progress toward Universal Health Coverage. Conclusion: AI-driven digital health technologies offer significant potential for strengthening healthcare systems and advancing Universal Health Coverage. However, effective implementation requires strong governance frameworks, ethical safeguards, and equitable access to digital technologies.
